Claims:

You can start a claim online. Once submitted, you are also able to check the status of your claim at your convenience.

Our Claims team can provide you with your Plan Number and assist you in starting your claim. Once your claim has been started, you will be able to check the status of your claim online where you will be asked to input your Plan Number and your Last Name.
